The county clerk shall preserve such affidavit in a bound volume and shall issue to every licentiate duly regis- tering and making such affidavit, a certificate of registration in his county, which shall include a transcript of the regis- tration. Such transcript and license may be offered as primary evidence in all courts of the facts therein stated. A copy of such certificate of registration shall be sent by the county clerk to the secretary of the board within five days after it is made.
