When the fistulous track is hard and callous, it should be slit open and scraped, or cauterised, aud aify bands of callous, tibrous tissue between the rectum and bladder, or prostate, should be divided. Various rather extensive operations with the object of completely detaching the rectum from the prostate, and closing each of the openings into the bowel have been recom- mended or performed by Astley Cooper, Tiilaux, Quenu, and E. Forgue. Mr. T. Myles, of the Richmond Hospital, Dublin (Med, Pre...sa, Dec. 6, 1893), after establishing supra-pubic drainage, cut out the fistulous tracks and made a longitudinal incision in the roof of the urethra opposite the deficiency in its floor, and was thereby able to suture the cut edges of the floor together around a No. 8 catheter. The result was a complete success. When the urethra is wounded, as it easily may be in some cases, during excision of the rectum, the fistula rapidly closes as the healing of the wound progresses.
