(Reverse side of Enlistment Record) Physical examination may be made by medical officers of the Regular Army, National Guard officers. Reserve Corps, the Navy, U. S. Public Health Service, or by civilian physicians. Should none other than a civilian physician be available to make the physical examina- tion, it is required that the name or names of a reputable physician of the community be submitted promptly to the State Adjutant General, who will report the name to the National Guard Bureau tha...t he may be furnished a copy of the regulations prescribing the physical requirements for enlist- ment. The examining physician will be advised that Questions 5 6, Chest X-Ray, and 57, Serology, are not required to be completed except in a case in which the condition of the man would deem to warrant the answer to such questions. It is not deemed necessary to await receipt of the regula- tions mentioned to begin the physical examination of men. Section 9 of the Notes on Reorganization of Ground Units mentioned, pertaining to re- quirements for Federal recognition of a unit, should be read.
