It may be that some instances of chronic general nephritis are directly caused by a diminution of blood-supply due to arterio-sclerotic changes in the renal vessels. This is, however, certainly not the general cause, and it seems likely that the etiological relation, if any, of the two conditions is in the reverse direction, in the sense that the increase of the media and intima is due to a persistent rise of blood-pressure which is compensatory to arterial rigidity and CIRCULATION 313 Jack of ...vasomotor flexibility in the renal vessels, and which aims at driving the proper quantum of blood through the kidneys. In this connection it is perhaps peculiarly instructive that a similar rise of general blood-pressure is found in cases of hydronephrosis, in which there is a general obstruction to the renal circulation and a more or less considerable diminution in the bulk of secreting tissue. On the other hand, it is open to question whether there is any causative connection at all between hyperpiesis and arterio-sclerosis on the one hand, and chronic general nephritis on the other.
