Now, this diagram is designed chiefly to portray the mechanism of one peculiar symptom, and the use to which it may be employed by the neurologist in definitely determining the seat of the disease-lesion which is creating it. The following steps are commonly employed to detect the existence of this symptom : Request the patient to close one eye by pressing the lid down with the finger, and to so direct the open eye as to concentrate its gaze upon some fixed object near to it. [I usually hold up the forefinger of my own hand within a foot of the patient's open eye, and tell him to look steadily at it.] Having done this, take some object which is easily seen (such as a small piece of white paper) in the unem- ployed hand, and move it to the right and left of the object upon which the patient is gazing, and also above and below the object, asking the patient, in each case, if the two objects are seen simultaneously and with distinctness, and notice upon which side of the fixed object the patient can not i)erceive the moving object.
